We are so happy to be able to offer this program to our community! Please read the following acknowledgement prior to sending the registration form below:
· I understand that the Sunday School Breakfast Club is a free church program for children ages 4-13.
· I understand that children will be supervised by adult volunteers with valid Vulnerable Sector Checks.
· I will inform the church of any allergies, medical conditions, or other concerns that may affect my child’s participation.
· I understand that every reasonable effort will be made to provide a safe and caring environment.
· I understand that participation in games, crafts, and other activities involves ordinary childhood activity and minor bumps or scrapes may occur.
· I give permission for church leaders to seek emergency medical assistance for my child if I cannot be reached.
· I understand that I am required to pick up my child promptly at the required pickup time.
